• Adults (16 & over) - $110
• Children (Under 16) - $80
• Money Order, Cashier's Check or Personal Check payments ONLY (make payable to DEPARTMENT OF STATE)

• $25 per Passport (Cash, Money Order, Cashier's Check or Credit/Debit Card ONLY - NO Personal Checks)

• Two (2) 2x2 photos with light color (studio) background
• Certified copy of BIRTH CERTIFICATE
• Old passport or naturalization certificate
• CHILDREN under 16 MUST have a BIRTH CERTIFICATE. (Even with an old passport)
• NOTE! Children under 16 MUST have BOTH parents present. Children 16 - 18 MUST have at least one (1) parent present.
